Effective September 1, 2022, the reporting form is no longer in use.

September 8, 2020

Dear Rutgers Colleagues:

With the well-being of our faculty, staff and students as its highest priority, the university developed the Returning to Rutgers guide to ensure we begin to repopulate Rutgers spaces in a safe manner. As the 2020 fall semester begins, we take this opportunity to once again remind everyone that the health and safety of our community is a shared responsibility. While we are hopeful that our community will adhere to the requirements outlined in the guide and that most cases where they are not met will require a simple reminder between friends and colleagues, we recognize that some matters may need further review.

If you believe someone or some group at Rutgers is in violation of the established university safety guidelines, please fill out an online COVID Observation Reporting Form. Reports may be submitted anonymously. This form will not be monitored in real-time and has been developed to report only non-emergency concerns. Examples of the forms intended use include reporting:

  • issues with accessing university facilities;
  • an employee coming to work with COVID symptoms;
  • equipment/workspace sanitation concerns;
  • inappropriate mask or face coverings usage;
  • quarantine concerns;
  • social distancing;
  • a student coming to class sick;
  • an unauthorized event; and/or
  • an unauthorized guest(s) on campus.

If this is an emergency, please dial 9-1-1. If you are reporting a non-emergency but require immediate assistance, please contact the Rutgers Police Department at 732-932-7211.

Stay safe and be well,
